By accessing and using the services provided by Elevate IV Solutions ("we," "our," or "Company"), you agree to be bound by these Terms of Service. If you do not agree to these terms, please do not use our services.
2. Services Description
Elevate IV Solutions provides mobile IV hydration and vitamin infusion therapy services in the Phoenix Metro Area. Our services are administered by licensed Registered Nurses (RNs) and are intended for general wellness purposes.
3. Medical Disclaimer
Important: Our IV therapy services are not intended to diagnose, treat, cure, or prevent any disease. IV therapy is not a substitute for professional medical advice, diagnosis, or treatment. Always seek the advice of your physician or other qualified health provider with any questions you may have regarding a medical condition.
Before receiving any IV therapy treatment, you will be required to complete a health questionnaire and consent form. Our nursing staff may refuse service if they determine that treatment would not be appropriate for you.
4. Eligibility
You must meet the following requirements to use our services:
- Be at least 18 years of age (or have parental/guardian consent)
- Provide accurate health information
- Not have contraindications to IV therapy
- Be located within our service area at the time of service
5. Booking and Cancellation Policy
Booking: Appointments can be made through our website, phone, or mobile app. A valid payment method is required to secure your booking.
Cancellation: We require at least 24 hours notice for cancellations. Cancellations made less than 24 hours before the appointment may be subject to a cancellation fee of up to 50% of the service cost.
No-Shows: Failure to be present at your scheduled appointment time may result in a full charge for the service.
6. Payment Terms
Payment is due at the time of service unless otherwise arranged. We accept major credit cards, debit cards, and select insurance plans for qualifying treatments.
Prices are subject to change without notice. Any promotional pricing or discounts are valid only during the specified promotional period.
7. Informed Consent
Before receiving treatment, you will be asked to sign an informed consent form that outlines the potential risks and benefits of IV therapy. By signing, you acknowledge that you understand and accept these risks.
8. Liability Limitations
To the fullest extent permitted by law, Elevate IV Solutions shall not be liable for any indirect, incidental, special, consequential, or punitive damages arising out of or related to your use of our services.
Our total liability for any claim arising from our services shall not exceed the amount you paid for the specific service giving rise to the claim.
9. Client Responsibilities
You agree to:
- Provide accurate and complete health information
- Inform us of any changes to your health status
- Follow pre and post-treatment instructions provided by our staff
- Provide a safe and appropriate location for mobile services
- Treat our staff with respect and courtesy
